New Patient Registration

If you wish to register with the practice and live within our practice boundaries please call into the surgery and collect the relevant forms. We will need you to attend with photo ID (Passport, driving license) proof of address (utility bill etc) and your visa if you are visiting the country.  

You will be registered with the practice but have the right to choose a GP.

You will need to complete a new patient questionnaire (basic information about yourself and your medical history) and should you wish to have a health check please make an appointment with the healthcare assistant. (The check consists of checking your blood pressure, height, weight, and medical history).

Children under five years of age can be registered for child health surveillance. In addition you will be asked to provide details of childhood immunisations of children under six (this will ensure your child is included in the computerised recall system).

Medical treatment is available from the date of registration.  Please contact reception for further information.
